Please log into ADP , then go to the Myself tab>Pay>Tax Withholdings. In ADP you will see your Federal and State W4 forms where you can edit your withholdings. If you are increasing your life insurance by $20,000 or more, you will need to fill out a medical certification form called a Statement of Health.
