PFML Resources Process Whenever practicable, you must notify the Office of Human Resources at least 30-days notice of PFML leave. Below is the necessary paperwork that needs to be submitted to the Office of Human Resources: Request for Paid Family & Medical Leave (to be completed by employee) MetLife Healthcare Provider Form (to be completed by Healthcare Provider*) The Office of Human Resources is here to support you with any questions regarding leaves. In addition, if you need any help with filling out your medical leave forms, TouchCare is here to support. Once you have submitted your completed forms, the Office of Human Resources and the Payroll Team will work together to create a financial calendar based on estimates. Once your leave begins, we will receive notification from MetLife to finalize your adjustments if necessary. Returning from leave: Prior to returning to work from a PFML leave for an employee’s serious health condition, other than pregnancy and birth, the employee will be required to submit a fitness for duty certification from their health care provider. If there are any salary adjustments made based on the length of leave, the Office of Human Resources and the Payroll Team will notify you. It is recommended that you meet with The Office of Human Resources and the Payroll Team to review the adjustments and provide any clarity.
